Your First Office Visit (Consultation)

Your initial office appointment will consist of a consultation with Dr. Bilovsky to evaluate you and determine your treatment needs. A complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Patients requesting or requiring IV sedation or general anesthesia require a pre-operative evaluation except in certain emergency situations.

X-Rays: If your dentist or physician has taken x-rays, please pick them up and bring them to our office. Even though you may have recent routine dental films, we frequently need to take a special x-ray called a panoramic x-ray or Panorex to more carefully evaluate your teeth and the supporting jawbone. If additional films are necessary, we will take them at our office.


Please assist us by providing the following information before or at the time of your consultation:

  • A surgical referral slip from your dentist or physician indicating why you are seeing us
  • Bring any X-rays which were given to you
  • A list of medications you are presently taking
  • If you would like to register online through our secure server, click here. You can also fill out the form, print it, and bring it to our office.
  • All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit and all subsequent visits.

  • Form of Identification

 

Appointment Scheduling

Our offices are open 5 days a week, Monday through Friday. Please call us at (818) 789-6622 to schedule an appointment.

Consultation appointments are usually in the afternoon as the morning appointments are reserved for our surgical patients. We will make every attempt to schedule your appointment in a timely and convenient manner. If you anticipate a need for your treatment to be done shortly after your consultation, please let our Treatment Coordinators know, as it may be possible to schedule your surgical appointment at the time your consultation appointment is scheduled.

We try our best to stay on schedule and see you at the scheduled time. Due to the fact that we provide surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

Our Financial Policy

We are participating members of many insurance companies, including (but not limited to) Blue Cross/Blue Shield, Delta, Delta PMI, Ameritas, Cigna, Aetna, Metlife and many others.

You will be given, at the time of your consultation, a treatment estimate. This estimate will outline the fees associated with your treatment as well as the estimated insurance payment. Every attempt will be made to maximize your dental benefits and in many cases, only a small co-payment is due at the time of surgery. Each estimate will be as accurate as possible based on the information received from you and the carrier. All accounts 90 days or older will be collected from the patient regardless of insurance claim status. We will however continue to supply services for your direct reimbursement from your carrier.

For those patients whose dental fees are not covered by insurance, we can assist you in obtaining low cost financing with Capital One and Care Credit without delaying your treatment.

For your convenience, we accept Visa, MasterCard and American Express. We also accept cash or a personal check with valid driver’s license.

 

Emergency Appointments

We recognize that many times it is not possible to anticipate needing our services. Many dental emergencies, especially toothaches, arise during the night. If you awake in pain or have an emergency situation, every attempt will be made to see you the following weekday. If your dentist is unavailable, please call our office between 8:30 and 10:00am in order to make an emergency appointment. Payment in full is expected for emergency care regardless of insurance coverage.

If you have a new dental emergency over the weekend, such as a loose filling or toothache, please call your dentist first. Usually, definitive dental care is not available after hours and on weekends for routine dental problems due to facility and staffing availability. Your local hospital emergency room can usually prescribe medication (after seeing you for evaluation) to keep you comfortable until your dentist can see you.

All current patients of  the Oral and Implant Surgery Center with postoperative problems or concerns will be able to speak with the doctor directly, regardless of the day or time.
